182·中国CT和MRI杂志2023年02月第21卷第02期总第160期【第一作者】文小检,男,主治医师,主要研究方向:中枢神经系统MR诊断及应用。E-mail:76565495@qq.com【通讯作者】文小检短篇论著ApplicationofLow-FieldMRIintheDiagnosisofIntramuralPregnancy(Reportof5Cases)WENXiao-jian1,2,*,SHENGuang-wu2.1.DepartmentofRadiology,HunanChestHospital,Changsha410013,China2.DepartmentofRadiology,HunanProvincialCorpsHospitalofCAPF,Changsha410000,ChinaABSTRACTObjectiveToexploretheMRImanifestationsanddiagnosticvalueofintramuralpregnancyintheuterus,andtoprovideabasisfortheearlydiagnosis,accuratepositioningandselectionoftreatmentoptionsforthistypeofdisease.MethodsTheclinicaldataof5casesofintramuscularpregnancydiagnosedbysurgeryandpathologyinourhospitalwerecollected,mainlyincludingage,menopausetime,relevantlaboratoryexaminations,gynecologicalhistory,ultrasoundexaminationandotherdata.AndtheMRIimagingdataofthelesion(thelocationofthelesion,sizeandshape,internalsignals,surroundingstructuresandothermanifestations)wereobservedtoanalyzetheMRIimagingcharacteristicsofthelesion.ResultsOfthe5patientswithintramuralpregnancy,4caseshadahistoryofmultipleuterinecavityoperations(Abortionorcurettage),and1casehadpelvicsofttissueadhesions.Thelesionsin3caseswerelocatedontheposteriorwall,1casewaslocatedonthelateralwall,and1casewaslocatedontheuterinecorner.Thelesionswereroundorquasi-circular,withasizeof(2.24cm×1.40cm×1.40cm)~(2.72cm×2.70cm×2.13cm).Thesignalinthelesionwasuneven,manifestedasmixedsignal.And,therewerevesicle-likehypo-intensityonT1WI,hyper-intensityonT2WI,whichwereseeninthelesions.Thepatchyhyper-intensitywasseenaroundthelesiononT1WIin2cases,whichindicatedhemorrhage.Themainbodyofthelesionislocatedintheuterinewall,theendometrialhyper-intensityandthejointzonearecontinuous,andthehyper-intensityinthemyometriumisinterrupted,showingswellinggrowthwithoutbreakingthroughtheserosallayer.ConclusionMRIcanprovideearlydiagnosisandaccuratepositioningofintramuralpregnancy.Ifitisdifficulttodiagnosebyclinicalandultrasound...
