临床医学研究与实践2023年9月第8卷第26期DiagnosticvalueofDCE-MRIcombinedwithDWIexaminationsinbenignandmalignantmusculoskeletaltumorsYANGChao1,ZHANGShaobin1,CHENLili1*,ZHANGMi2(1.TraditionalChineseMedicineHospitalofBaoji,Baoji721000;2.theFirstAffiliatedHospitalofXi'anJiaotongUniversity,Xi'an710061,China)ABSTRACT:ObjectiveToexplorethediagnosticvalueofdynamiccontrastenhancedmagneticresonanceimaging(DCE-MRI)combinedwithdiffusionweightedimaging(DWI)examinationsinbenignandmalignantmusculoskeletaltumors.MethodsAtotalof98patientswithmusculoskeletaltumorsfromJanuary2021toJanuary2022wereselectedasthestudysubjects.AllpatientsunderwentDCE-MRIandDWIexaminations.ThediagnosticvalueofDCE-MRI,DWIindependentandcombinedexaminationsinbenignandmalignantmusculoskeletaltumorswasanalyzed.ResultsPathologicalexaminationconfirmedthatamongthe98patientswithmusculoskeletaltumors,58cases(59.18%)weremalignanttumorsand40cases(40.82%)werebenigntumors.InDWIexamination,theapparentdiffusioncoefficient(ADC)oftheparenchymalpartofbenigntumorswithdifferentdiffusionsensitivegradient(b)valueswerehigherthanthoseofmalignanttumors(P<0.05).InDCE-MRIexamination,thevolumetransferconstant(Ktrans),rateconstant(Kep)andextravascularextracellularvolumefraction(Ve)ofbenigntumorswerelowerthanthoseofmalignanttumors(P<0.05).Theresultsofreceiveroperatingcharacteristic(ROC)curveshowedthatinDWIexamination,thediagnosticefficiencywasthebestwhenb=1200s/mm2,andtheareasundercurve(AUC)forthediagnosisofmalignantmusculoskeletaltumorswas0.937,thesensitivitywas89.76%,andthespecificitywas88.00%;intheDCE-MRIexamination,thediagnosticefficiencyofKtransparameterwasthebest,andtheAUCforthediagnosisofmalignantmusculoskeletaltumorswas0.891,thesensitivitywas89.80%,andthespecificitywas83.00%;inDWIcombinedwithDCE-MRIexamination,theAUCof1200s/mm2+Ktransparameterinthediagnosisofmalignantmusculoskeletaltumorswas0.965,thesensitivitywas82.86%,andthespecificitywas92.50%.ConclusionDCE-MRIcombinedwithDWIexaminationsinbenignandmalignantmusculoskeletalDCE-...
