中草药 中草药 2023 年年 3 月月 第第 54 卷卷 第第 5 期期 Chinese Traditional and Herbal Drugs 2023 March Vol.54 No.5 1511 湘 A1 颗粒改善脾虚湿盛证 HIV/AIDS 患者免疫功能的临床疗效及机制研究2 谭 瑶1，王军文1*，邓玉霞1，龚 霞1，谢小丽1，张予晋2，刘 坤3 1.湖南中医药大学，湖南 长沙 410208 2.湖南中医药大学第二附属医院，湖南 长沙 410005 3.益阳市第四人民医院，湖南 益阳 413000 摘 要：目的 探讨湘 A1 颗粒改善脾虚湿盛证人类免疫缺陷病毒（human immunodeficiency virus，HIV）/获得性免疫缺陷综合征（acquired immunodeficiency syndrome，AIDS）患者免疫功能的临床疗效及其作用机制。方法 采用随机数字表法将符合要求的54例HIV/AIDS脾虚湿盛证患者分为治疗组28例，对照组26例。治疗组采用高效抗反转录病毒疗法（highly activeantiretroviral therapy，HAART）联合湘 A1 颗粒进行治疗，对照组采用 HAART 联合湘 A1 颗粒模拟剂治疗。两组均连续治疗 6 个月，分别于治疗前后评估两组患者临床疗效、症状体征积分、生存质量分值，比较CD4+T 细胞计数以及血清转化生长因子-1（transforming growth factor-1，TGF-1）、白细胞介素-10（interleukin-10，IL-10）、干扰素（interferon-，IFN-）、肿瘤坏死因子-（tumor necrosis factor-，TNF-）水平，并对安全性指标进行评价。结果 治疗后，治疗组总有效率（92.86%）明显高于对照组（69.23%）（P0.05）；治疗后，两组患者症状体征积分、TGF-1、IL-10 水平均较治疗前均明显下降（P0.05），且治疗组较对照组下降更为明显（P0.05）；两组治疗后生存质量分值、CD4+T 细胞计数、IFN-水平均较治疗前明显提高（P0.05），且治疗组明显高于对照组（P0.05）。两组均未出现明显不良反应。结论 湘A1 颗粒治疗脾虚湿盛证 HIV/AID 患者临床疗效显著，可以有效改善艾滋病患者症状、提高生存质量、促进免疫功能。其作用机制可能与调节血清细胞因子TGF-1、IL-10、IFN-水平相关。关键词：湘 A1 颗粒；人类免疫缺陷病毒；艾滋病；免疫功能；转化生长因子-1；白细胞介素-10；干扰素 中图分类号：R285.64 文献标志码：A 文章编号：0253-2670(2023)05-1511-06 DOI:10.7501/j.issn.0253-2670.2023.05.018 Clinical observation on promoting immune function of Xiang A1 Granules and its mechanism in HIV/AIDS patients with spleen deficiency and dampness excess TAN Yao1,WANG Jun-wen1,DENG Yu-xia1,GONG Xia1,XIE Xiao-li1,ZHANG Yu-jin2,LIU Kun3 1.Hunan University of Chinese Medicine,Changsha 410208,China 2.The Second Affiliated Hospital of Hunan University of Chinese Medicine,Changsha 410005,China 3.The Fourth Peoples Hospital of Yiyang City,Yiyang 413000,China Abstract:Objective To explore the clinical efficacy and mechanism of Xiang A1 Granules(湘 A1 颗粒)in promoting immune function in human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS)patients with spleen deficiency and dampness excess syndrome.Methods A total of 54 cases of HIV/AID patients with spleen deficiency and damp excess syndrome were randomly divided into treatment group(28 cases)and control group(26 cases).The treatment group was treated with highly active antiretroviral therapy(HAART)combined with oral Xiang A1 Granules,while the control group was treated with HAART combined with Xiang A1 Granules mimicking agent.Both groups were treated continuously for six months.The clinical efficacy,the scores of symptoms and signs,the scores of quality of life,the CD4+T cell count,and the expression levels of transforming growth factor-1(TGF-1),interleukin-10(IL-10),interferon-(IFN-),tumor necrosis factor-(TNF-)of the patients in two groups were evaluated before and after treatment.And the safety indicators were evaluated.Results After treatment,the total effective rate of the treatment group(92.86%)was significantly higher than that of the control group(69.23%)(P 0.05).The symptoms and signs scores,TGF-1 and IL-10 levels of the patients in two groups after treatment were significantly decreased compared with those before 收稿日期：2022-07-25 基金项目：湖南省自然科学基金项目（2022JJ30442）；湖南省中医药科研计划项目（202011）；湖南省教育厅研究项目（20A382）；湖南中医药大学研究生科研创新课题（2021CX300）作者简介：谭 瑶，在读博士研究生，研究方向为中医药防治艾滋病。E-mail:*通信作者：王军文，教授，从事中医药防治艾滋病研究。E-mail:wangjunwen_ 1512 中草药 中草药 2023 年年 3 月月 第第 54 卷卷 第第 5 期期 Chinese Traditional and Herbal Drugs 2023 March Vol.54 No.5 treatment(P 0.05),and the decrease was more significant in the treatment group compared with the control group(P 0.05).The scores of quality of life,CD4+T cell count,and IFN-levels were significantly increased(P 0.05),and the treatment group was significantly higher than the control group(P 0.05).There were no obvious adverse reactions in both groups.Conclusion Xiang A1 Granules have significant clinical efficacy in the treatment of HIV/AID patients with spleen deficiency and dampness excess,which can effectively improve the symptoms of HIV/AID patients,improve the quality of life,and promote immune function.Its mechanism of action may be related to the regulation of serum TGF-1,IL-10 and IFN-cytokines.Key words:Xiang A1 Granules;human immunodeficiency virus;acquired immunodeficiency syndrome;immune function;TGF-1;IL-10;IFN-获得性免疫缺陷综合征（艾滋病，acquired immunodeficiency syndrome，AIDS）作为一种威胁人类生命健康的传染性疾病，针对其治疗策略的探索从未停止。高效抗反转录病毒疗法（highly activeantiretroviral therapy，HAART）可以有效减少人类免疫缺陷病毒（human immunodeficiency virus，HIV）的复制，降低艾滋病死亡率，但无法使艾滋病得到根本治愈。由于稳定的病毒感染细胞储库持续存在，HAART 中断容易导致病毒血症的快速反弹，或引发不良反应1-2。近年来，中医药积极参与艾滋病防治研究进程，在解除患者痛苦、降低抗病毒不良反应、提高临床疗效等方面发挥了积极作用。湘A1 颗粒是根据湖南省艾滋病患者体质研制的中药方剂，由山药、茯苓、藿香、茵陈、连翘、石菖蒲、白蔻仁、虎杖、滑石、川木通、薄荷、薏苡仁、白花蛇舌草、甘草组成，治疗艾滋病已 10 余年，取得了确切的临床效果。前期研究证实，其在提升免疫缺陷型小鼠 T 细胞亚群、提高脾虚湿盛证艾滋病患者临床疗效等方面作用显著，并具有较好的安全性3-4，但具体作用机制尚未明确。各类免疫细胞分泌的细胞因子对清除炎症介质、减轻感染、维持免疫系统稳态、改善免疫功能等方面具有重要影响5，细胞因子疗法是艾滋病治疗策略的重要途径。因此，本研究通过临床观察，明确湘 A1 颗粒促进 HIV/AID患者免疫水平的疗效。同时，以转化生长因子-1（transforming growth factor-1，TGF-1）、白细胞介素-10（interleukin-10，IL-10）、干扰素（interferon，IFN-）、肿瘤坏死因子-（tumor necrosis factor-，TNF-）4 种重要细胞因子为切入点，旨在探寻湘 A1 颗粒治疗艾滋病发挥免疫调节作用的具体机制，为湘 A1 颗粒的进一步推广应用提供依据。1 资料与方法 1.1 一般资料 选取 2020 年 4 月2021 年 12
