临床医学研究与实践2023年3月第8卷第9期Effectsofhospital-community-familyhealthmanagementmodelcombinedwithknowledge-attitude-practiceinterventiononhealthcognition,behaviorandbloodpressurecontrolinelderlypatientswithhypertensionXUEJuan,LEIMingxia*(YulinNo.2Hospital,Yulin719000,China)ABSTRACT:ObjectiveToexploretheapplicationeffectofhospital-community-familyhealthmanagementmodelcombinedwithknowledge-attitude-practiceinterventioninelderlypatientswithhypertension.MethodsAtotalof200elderlypatientswithhypertensionadmittedinourhospitalfromMarch2020toMarch2021wereselectedanddividedintocontrolgroup(100cases)andobservationgroup(100cases)byrandomnumbertablemethod.Thecontrolgroupwasgivenroutinenursing,andtheobservationgroupwasgivenhospital-community-familyhealthmanagementmodelcombinedwithknowledge-attitude-practiceinterventiononthebasisofthecontrolgroup.Theinterventioneffectsofthetwogroupswerecompared.ResultsAfterintervention,thescoresoftheHypertensionKnowledge-LevelScale(HK-LS),theHypertensionPatientsSelf-ManagementBehaviorRatingScale(HPSMBRS)intheobservationgroupwerehigherthanthoseinthecontrolgroup(P<0.05).Afterintervention,thefacescoreintheobservationgroupwashigherthanthatinthecontrolgroup,andtheavoidanceandyieldscoreswerelowerthanthoseinthecontrolgroup(P<0.05).Afterintervention,themorningpeakofsystolicbloodpressureanddiastolicbloodpressureintheobservationgroupwerelowerthanthoseinthecontrolgroup(P<0.05).ConclusionThehospital-community-familyhealthmanagementmodelcombinedwithknowledge-attitude-practiceinterventionforelderlypatientswithhypertensioncanhelpimprovehealthcognitionandbehavior,improvecopingstyles,andpromotetheeffectiveimprovementofbloodpressurecontrol.Itisworthpromoting.KEYWORDS:hospital-community-familyhealthmanagementmodel;knowledge-attitude-practiceintervention;elderly;hypertension医院-社区-家庭健康管理模式结合知信行干预对老年高血压患者健康认知、行为及血压控制的影响薛娟，雷明霞*（榆林市第二医院，陕西榆林，719000）摘要：目的探讨医院-社区-家庭健康管理模式结合知信行...
