《中国病案》2023年第24卷第7期·76·冠脉支架降价前后患者住院费用及影响因素分析黎珍君①覃华凌①陆丹艳①林书豪①*摘要目的分析冠脉支架降价前后患者住院费用构成及影响因素，为进一步降低冠脉支架置入术患者住院费用提供参考。方法选取广西某三级甲等综合性医院2020年1月1日-2021年12月31日出院的2172例冠脉支架置入术患者作为研究对象，其中冠脉支架降价前出院患者数为913例，降价后出院患者数为1259例,收集患者基本信息、诊疗信息和费用信息数据，采用描述性分析、单因素分析和多元线性逐步回归分析方法对住院费用的影响因素进行统计分析。结果冠脉支架降价后患者例均费用从43199.84元下降至24132.77元，其中卫生材料费下降19.93%，检验检查费、治疗费、西药费和护理费上升较为明显，分别为8.22%、4.96%、3.18%、2.9%。多因素分析发现影响冠脉支架降价后患者住院费用的主要因素按照标准化回归系数绝对值的大小排序依次是出院年份和住院天数。结论冠脉支架降价后患者经济负担明显减轻。住院费用中卫生材料费下降幅度最大，检验检查费上升幅度最大。出院年份和住院天数是冠脉支架置入术患者住院费用的主要影响因素。关键词冠脉支架；住院费用；影响因素；集采；高值医用耗材AnalysisofHospitalizationCostsandInfluencingFactorsofPatientsBeforeandAfterCoronaryStentPriceReductionLiZhenjun,QinHualing,LuDanyan,LinShuhaoAbstractObjectivesThisstudyaimstoanalyzethecompositionandinfluencingfactorsofpatients'hospitalizationexpensesbeforeandafterthepricereductionofcoronarystentsandtoprovideareferenceforfurtherreducingthehospitalizationexpensesofpatientsundergoingcoronarystenting.MethodsAtotalof2172patientswithcoronarystentswhoweredischargedfromatertiarygeneralhospitalinGuangxifromJanuary1,2020toDecember31,2021wereselectedastheresearchsubjects.Amongthem,913patientsweredischargedbeforethepricereductionofcoronarystents,and1259patientsweredischargedafterthepricereduction.Patientbasicinformation,diagnosisandtreatmentinformation,andcostinformationdatawerecollected.Descriptiveanalysis,univariateanalysis,andmultiplelinearstepwiseregressionanalysismethodswereusedtostatisticallyanalyzetheinfluencingfactorsofhospitalizationexpenses.ResultsAfterthepricereductionofcoronaryste...
