第 49 卷 第 2 期2023年 3 月吉林大学学报(医学版)Journal of Jilin University(Medicine Edition)Vol.49 No.2Mar.2023DOI:10.13481/j.1671587X.20230226发育性语言障碍共病注意缺陷多动障碍患儿血清维生素 D水平检测及其意义张延赤1,张晓敏1,王艳波1,袁久彤1,张晓威2,吴秀丽3,马英伟4,田玉玲4,王利武4,孙利伟4(1.吉林省长春市儿童医院心理科,吉林 长春 130061;2.吉林省长春市儿童医院检验科,吉林 长春130061;3.吉林省长春市儿童医院康复科,吉林 长春 130061;4.吉林省长春市儿童医院科研室,吉林 长春 130061)摘要 目的目的:检测发育性语言障碍(DLD)共病注意缺陷多动障碍(ADHD)患儿外周血维生素 D(VD)水平及存在形式,阐明 VD 在 DLD 共病 ADHD 发生中的作用,为其治疗提供新的方法。方法方法:选择就诊的 DLD共病 ADHD患儿 90例为观察组,以 50名同期在本院健康体检中心体检的身心健康儿童为对照组。DLD 和 ADHD 诊断参照 国际疾病分类 第 11次修订本(ICD-11)和 美国精神障碍诊断及统计手册 第 5版(DSM5)相关章节,采用格里菲斯精神发育量表(GMDS)语言能分量表测评并计算语言发育商(DQ),根据斯诺佩评定量表(SNAP-4)家长及教师问卷进行ADHD 分型,采用 Conners父母症状问卷(PSQ)进行 ADHD 评分,采用液相色谱质谱法检测 2组研究对象血清中 VD 水平。收集所有研究对象 GMDS 和 PSQ 及观察组患儿 SNAP-4 测评结果;测试者DQ 小于 1001标准差(SD)为语言能力低于同龄人;根据 PSQ 将儿童行为问题分为学习问题、心身问题、焦虑、多动-冲动、品行问题和多动指数共 6种因子,6种因子得分均大于 2分可诊断存在相关问题;根据 SNAP-4 将 ADHD 分为注意缺陷为主型(ADHD-I)、多动/冲动型为主型(ADHD-HI)和混合型(ADHD-C)。结果结果:观察组男性占 71.1%,女性占 28.9%。观察组患儿血清中 VD3和VD 水平明显低于对照组(P0.01);观察组 DLD-ADHD-C、DLD-ADHD-HI和 DLD-ADHD-I 3 种类型患儿 VD 和 VD3水平均明显低于对照组(P0.01)。对照组研究对象 DQ 值与血清中 VD 和 VD3水平呈正相关关系(r=0.512,P0.01;r=0.529,P0.01);观察组患儿 DQ 值与血清中 VD 和VD3 水平呈正相关关系(r=0.299,P0.01;r=0.279,P0.01),与 VD2 水 平 呈 负 相 关 关 系(r=0.122,P0.01)。观 察 组 DLD-ADHD-C、DLD-ADHD-HI 和 DLD-ADHD-I 3 种 类 型 患 儿PSQ 测评学习问题、心身问题、品行问题、焦虑、多动/冲动和多动指数分值与对照组比较差异有统计学意义(P0.01)。血清中 VD3水平与 DLD-ADHD-C 型观察组患儿上述 6种因子测评分值均呈明显负相关关系(r=0.438,r=0.357,r=0.422,r=0.465,r=0.583,r=0.593,P0.01),与 DLD-ADHD-HI型观察组患儿品行问题呈正相关关系(r=0.522,P0.01),与多动/冲动和多动指数呈明显负相关关系(r=0.455,P0.05;r=0.424,P0.01),与心身问题和多动/冲动呈负相关关系(r=0.468,r=0.496,P0.05),与 多 动 指 数 呈 正 相 关 关 系(r=0.694,P0.01)。VD 水平与 DLD-ADHD-C 型观察组患儿上述 6种因子测评分值均呈明显负相关关系(r=0.444,r=0.498,r=0.450,r=0.501,r=0.594,r=0.522,P0.01),与 DLD-ADHD-HI型观察组患儿心身问题、品行问题、多动/冲动和多动指数均呈负相关关系(r=0.355,r=0.578,r=0.509,r=0.422,P0.05或 P0.01),与心身问题和多动/冲动呈负相关关系(r=0.485,r=0.497,P0.05),与 多 动 指 数 呈 明 显 正 相 关 关 系(r=0.682,P0.01)。结论结论:VD3缺乏可能是 DLD 共病 ADHD 的病因之一。补充 VD3可能对预防和治疗 DLD 共病 ADHD患儿有积极作用。文章编号 1671587X(2023)02049209收稿日期 20220513基金项目 吉林省卫健委科技创新项目(2021LC110)作者简介 张延赤(1969),男,吉林省长春市人,主任医师,教授,医学硕士,主要从事医学心理学方面的研究。通信作者 孙利伟,主任医师(E-mail:)492张延赤,等.发育性语言障碍共病注意缺陷多动障碍患儿血清维生素 D水平检测及其意义关键词 语言障碍;注意缺陷多动障碍;共病;维生素 D中图分类号 R749.94文献标志码 ADetection of serum vitamin D levels of children with developmental language disorder comorbid with attention deficit hyperactivity disorder and its significance ZHANG Yanchi1,ZHANG Xiaomin1,WANG Yanbo1,YUAN Jiutong1,ZHANG Xiaowei 2,WU Xiuli3,MA Yingwei4,TIAN Yuling4,WANG Liwu4,SUN Liwei4(1.Department of Psychology,Children s Hospital,Changchun City,Jilin Province,Changchun 130061,China;2.Department of Clinical Laboratory,Children s Hospital,Changchun City,Jilin Province,Changchun 130061,China;3.Department of Rehabilitation,Children s Hospital,Changchun City,Jilin Province,Changchun 130061,China;4.Department of Scientific Research,Children s Hospital,Changchun City,Jilin Province,Changchun 130061,China)ABSTRACT Objective:To detect the levels and existing forms of vitamin D(VD)in peripheral blood of the children with development language disorder(DLD)comorbid with attention deficit hyperactivity disorder(ADHD),to clarify the role of VD in the occurrence of DLD comorbid with ADHD,and to provide the new method for its treatment.Methods:A total of 90 children with DLD comorbid with ADHD were regarded as observation group,and fifty physically and mentally healthy children in the Health Examination Center in our hospital during the same period were selected as control group.The DLD and ADHD were diagnosed according to the relevant chapters of the International Classification of Diseases,11th Revision(ICD-11)and the American Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition(DSM-5).Griffiths Mental Development Scales(GMDS)was used to detect the language ability and the Developmental Quotient(DQ)was caculated;ADHD was classified according to the Snoopy Rating Scale (SNAP-4)Parent and Teacher Questionnaire;Conners Parent Symptom Questionnaire(PSQ)was used for ADHD scoring;the levels of serum VD of the subjects in two groups were detected by liquid chromatography-mass spectrometry.The results of GMDS and PSQ of all subjects and SNAP-4 of the subjects in observation group were collected.When DQ was less than(1001)standard deviation,the language ability of the subjects was lower than that of the contemporary;the children s behavioral problems were divided into six factors according to the PSQ,such as learning problem,psychosomatic problem,anxiety,hyperactivity-impulsivity,conduct problem,and hyperactivity index;when the scores of six factors were all greater than 2 points,the related problems could be diagnosed;according to SNAP-4,ADHD was divided into predominantly inattentive ADHD(ADHD-I),predominantly-hyperactivity/impulsivity(ADHD-HI)and combined ADHD(ADHD-C)groups.Results:In observation group,71.1%were male and 28.9%were female.The levels of serum VD3 and VD of the chinldren in observation group were significantly lower than those in control group(P0.01).The levels of serum VD and VD3 o
