Designation:F 1305 94(Reapproved 2002)Standard Guide forClassification of Amusement Ride and Device RelatedInjuries and Illnesses1This standard is issued under the fixed designation F 1305;the number immediately following the designation indicates the year oforiginal adoption or,in the case of revision,the year of last revision.A number in parentheses indicates the year of last reapproval.Asuperscript epsilon(e)indicates an editorial change since the last revision or reapproval.1.Scope1.1 This guide provides a uniform procedure that should beused when classifying patron injury and illness data related toamusement rides and devices.1.2 This standard does not purport to address all of thesafety concerns,if any,associated with its use.It is theresponsibility of the user of this standard to establish appro-priate safety and health practices and determine the applica-bility of regulatory limitations prior to use.2.Terminology2.1 Definitions of Terms Specific to This Standard:2.1.1 illness,npersonal discomfort resulting in treatmentincluding a personal illness,food poisoning,drug abuse,toxicinhalation,insect sting,or other similar occurrence.2.1.2 injury,nsustained bodily harm resulting in treat-ment such as trauma,cuts,bruises,burns,and sprains.2.1.3 minor injuries/illnesses,ninjuries and illnesseswhich may or may not require emergency first aid or significanttreatment,or both,but cannot be otherwise classified as aserious injury or illness.This category includes incidentswhere treatment is limited to such things as the dispensation ofover-the-counter medication or plastic adhesive strips2,cleans-ing,rest,and other similar duties or assistance.2.1.4 serious injuries/illnesses,na personal injury/illnessthat results in death,dismemberment,significant disfigure-ment,permanent loss of the use of a body organ,member,function,or system,a compound fracture,or other significantinjury/illness that requires immediate admission and overnighthospitalization and observation by a licensed physician.3.Significance and Use3.1 The purpose of this guide is to provide a uniformprocedure under which the amusement ride and device industrycan organize data related to injuries and illnesses.This classi-fication system may be used to formulate statistical informationwithin the categories provided and will facilitate the analysis ofinjury and illness incidents.The classification system mayassist owner/operators and manufacturers to review incidentsdirectly related to their amusement rides and devices,and mayprovide information for alternatives to reduce or eliminatesimilar occurrences.4.Recording Recommendations4.1 The administration of emergency health care service andtreatment should be recorded as deemed appropriate by theowner/operator of amusement rides and devices to include thedocumentation of all first-aid treatment,including minor inju-ries and illnesses,in a first-aid log.Injuries and illnesses otherthan minor should be reported on a first-aid incident report inaccordance with 4.2.4.2 First-Aid Incident ReportA first-aid incident reportshould be completed for injuries or illnesses that result inhospital admission or where medical treatment is given,recommended,or may be required at a future date.All injuriesor illnesses reported and other than those classified as minor,can be presumed to be in this category.4.3 Recorded Information:4.3.1 Information recorded in the first-aid incident reportshould include but not be limited to the following,whereapplicable:4.3.1.1 Date the incident occurred.4.3.1.2 Name,address,and telephone number of the personto receive emergency health care service or treatment.4.3.1.3 Age of the person to receive emergency health careservice or treatment.4.3.1.4 Manufacturers name of the amusement ride ordevice where or on which the incident occurred.4.3.1.5 Description of the injury or illness.Physical descrip-tion of the injury or illness.Description of the events causingand related to the incident.4.3.1.6 Description of the first-aid service or treatmentadministered including medications given.4.3.1.7 Incident classification in accordance with Section 5.4.3.1.8 Additional information deemed necessary by theowner/operator.5.Classification of First-Aid Incidents5.1 When recording an applicable first-aidrelated
